Q: When can I start saving with my subscription?
A: Your subscription is effective immediately the day you purchase it.

Q: How do I schedule my preventive dental care?
A: Simply call, email, text, or chat with our office. Just tell us that you have a subscription and you’d like to make an appointment.

Q: What if I come in today for a cleaning and buy a subscription, will it apply to today’s dental services?
A: Absolutely! Memberships are effective as of the purchase date, even after treatment is already completed!

Q: Can I use my subscription for restorative, elective, or cosmetic dental services?
A: Yes, you save 20% off our fees for restorative, elective, or cosmetic dental services excluding Invisalign.

Q: What happens if I break an appointment?
A: A broken appointment is an appointment canceled, missed, or changed within 24-hours of the appointment time. You are allowed one broken appointment per subscription period. If you break your dental appointment a second time, you will forfeit the appointment with no refund given.

Q: Can I cancel my subscription?
A: Your subscription will automatically expire at the end of the 12 month period. You can cancel your subscription at any time, however no refunds will be given once a subscription period has started.

Q: Can I join before I come in for my appointment?
A: Of course. Keep in mind that the plan will expire 12 months from the subscription payment.

Q: Can I set my account to automatically renew?
A: Yes, you have the option to automatically have your account renew if you paid online with a credit or debit card.

Q: Will I be reminded before my subscription expires?
A: Yes, you will receive an email from our practice reminding you of your subscription’s expiration date.

Q: Will I receive an identification or subscription card?
A: Membership cards are not needed for subscriptions.

Terms & Conditions
To agree to our Terms & Conditions, you must be at least 18 years old. The Child Plan is for children age 17 years and younger. The Adult Plan is for adults age 18 years and older. The Perio Plan is required for anyone diagnosed with Periodontal Disease, as additional visits and treatment are required to treat the disease. Subscribers upgrading to the Perio Plan are handled on a case-by-case basis. All plan fees are subject to change.

Current patients enrolling in the program must have a zero ($0) balance. This discount program will not cover any referred services or any services performed outside this office, or treatment which, in the sole opinion of our doctors, lies outside the realm of their capability. 20% discount on all services except Invisalign. To receive 20% discount on treatment, payment is due on the day of service by check or cash. A 15% discount will apply if paying by credit or debit card, Health Savings card, or Flex Spending card. If unable to pay in full on the date of service, a financial arrangement must be made, and a 10% discount will apply. Discount cannot apply to any treatment started or completed prior to enrollment. This program is a pre-treatment discount program and not dental insurance. Program cannot be used with referrals to specialists, in conjunction with any other discounts or promotions, or for the purchase of dental products. Program may be used with dental insurance. Discounted fee will be reflected on your claim submitted insurance.

Enrollment fees are non-refundable. Your effective date is the day you sign up and your renewal date is the same date every consecutive year. Included preventive treatment that is not used during your 12-month enrollment period will be forfeited, will not carry over to next year, and cannot be transferred to any other person.

Appointments must be made more than 60-days prior to the end of your enrollment period to guarantee an appointment date in the enrollment period. Late afternoon and Saturday appointments are not guaranteed. If late afternoon or Saturday appointments are needed, pre-appointing your 6-month visit is highly recommended. A $100 reinstatement fee will apply if your plan lapses for any reason. A 7-day grace period will be given. Enrollments and benefits are not transferable to any other person. If enrollment is not renewed, future appointments may be cancelled if the office is unable to reach the patient to confirm the appointment.
